Our new office is Now Open at 1188 Call Creek Circle, Pocatello.

We are located in Southeast Idaho in the town of Pocatello, approximately 150 miles north of Salt Lake or 3.5 hours east of Boise.

Pocatello Creek Clinic - Exit 71 and turn West. Take your first left and left again into the office park.  We are the second building in the office park on the ground floor. (Click on image for map)
